At the bottom of this page, click on the underlined link for the scholarship instructions. Save or print the instructions. Click on the underlined link for the application - a form will appear that you can save to your computer and fill later, OR you can complete the form online and save it to your computer or print it. Suggestion: save the blank form to your computer and while filling it save the form every once in a while. That way, you will not lose your partially completed form if there is a problem. You can also go back later and make changes without having to start over.

Dates are to be entered as numbers only. They will be formatted appropriately. Enter as mmddyyyy.

• Example 05292023 will be displayed on the form as 05/29/2023

• If you open the document with the Chrome Browser, enter the dates as "MM/DD/YYYY". The Chrome browser does not support continuous string date entries. There are other programs that are similar.

 

Telephone numbers are to be entered as numbers only. They will be formatted appropriately.  Enter area code and telephone number as a continuous string like this: 8654443333.

• Example 865444333 will be displayed on the form as (865) 444-3333
